Your Membership plan includes:

- Complimentary Consultations
- Elective services, including cosmetic dentistry are included
This program is a discount plan, not a dental insurance plan. It cannot be used:
- In conjunction with another dental plan
- For dental services related to injuries covered under workman’s compensation
- For treatment which, in the sole opinion of the treating doctor, lies outside the realm of their capability
- For referrals to specialists
- For treatment rendered by outside specialists
- For costs of dental care which are covered under automobile/medical insurance
Program Guidelines
- Non-Refundable
- No refunds or premiums will be issued at any time if participant does not utilize the plan
- Patients’ portion of the bill is due at the time of service.
- The plan is in effect once the premium has been paid
